Our services are centered on the whole person and with this try to use all life domains to better quality of life. We remain directed by the individuals and families we serve, designing a care plan for and around all those who are important to the person

In home supports: homemaker and personal care 

A variety of services and supports designed by and for you and your family to keep you as independent as possible in your choice of home.

Examples of services include:

  • -Meal assistance and planning
  • -house and equipment cleaning
  • -assistance with personal care needs and medication supports
  • -training on completion of daily living and independent living skills
  • -protections against abuse, neglect, exploitation or other negative outcome risks
  • -positive supports for behavior changes or presentation
  • -planning for doctor's appointments, medications, and other health needs
  • -assist with basic medical tasks and services at appropriate level of training
  • -preparing and responding to emergencies if they arise including medical, safety, and life concerning emergencies

In community supports: personal supports and transportation

Supports provided to you while you navigate the community, wherever that may take you.

Examples include:

  • -Support with and transport to doctors appointments and obtaining needed prescribed medicines or equipment
  • -Transportation for essential shopping needs and other household services
  • -Travel to family, friends, clubs, and other social and spiritual settings
  • -Identifying and providing or obtaining needed supports in the community - including responsive actions to potential safety or health risks
  • -Encouragement to advocate and communicate for oneself
  • -Transportation to work or other regular day activities

Family and independent provider supports

Caregiving can be a very challenging experience and take its toll. And finding the right people to care for a loved one can be difficult if not feel almost impossible.

We are here to help in any way that we can including:

  • -supporting shared living and other family caregiving arrangements
  • -providing respite services to family caregivers and independent providers
  • -take the role of agency for you to give care but know your loved one can be taken care of if need be
  • Providing open communication regarding safety and outcomes with your loved ones with proper consent

Who do we serve?

 All individuals with disabilities or independent living assistance needs in Northeast Ohio.

We currently accept the following:

  • Medicaid waivers, specifically: individual options, Level one, and self waiver for individuals with a DD level of care
  • Private pay such as: trusts or STABLE account funds, long term care insurance, local sources of funding, out-of-pocket payments
